Slip and fall at the mall

I fell and sprained my ankle and elbow, and my tail. I went to the e/r.

I am not sure what to do next, I lost one day from work, but I suffer from lupus and the stress of the fall, resulted with me having a flare of lupus. how should I state my pain and suffering to the insurance company.
