Hitting someone who doesn't have a title
First thing if the person driving the other car doesn't have a title to that car (and they aren't borrowing it or stealing it) can they have insurance?
Second if you hit that person could your insurance rates go up?
Third assuming they aren't insured and you hit them can your insurance rate go up?

First, yes they can have insurance. If they are stealing the vehicle, that may void coverage, other than that, chances are they are able to get insurance.
Second, yes.
Third, yes. You hit a person (with or without), you are a higher risk to insured. The carrier will want o more money in premium for that newer documented risk.
